Entity Form

Description of Entity Form

Each form is divided into sections and each section can have a sub-section (up to a maximum of 3). A sub-section is essentially a section, but with a level number.
Each section has a two-part set-up:
- Control Setup for the type of response for the section
- Point Setup for the questions in the section
